The Family and Medical Leave Act

HOW SHOULD AN EMPLOYEE REQUEST FMLA LEAVE?
For staff, leave should be requested on a leave slip. There is space on the form where an employee can designate the leave as time off for purposes covered by the FMLA.

If a staff employee calls into work requesting time off for a reason apparently covered by the FMLA, the supervisor will complete the leave slip and designate the time off accordingly.
